When Melissa was referred to UConnect Coordination, her initial NDIS plan included funding of $45,189 to support her discharge from hospital. While this funding addressed some immediate needs, it did not adequately reflect the complexity of her circumstances or the level of support required for a safe and sustainable transition home.
Our Specialist Support Coordination team worked closely with Melissa, her family, hospital clinicians, allied health professionals, and the NDIS to develop a comprehensive discharge and support plan. We took the time to understand not only Melissa's immediate needs but also her long-term goals, daily support requirements, accommodation needs, assistive technology, and the services required to maximise her independence and quality of life.
Through detailed evidence gathering, collaboration with multidisciplinary teams, and strong advocacy, we ensured Melissa's needs were accurately represented to the NDIS.
As a result, Melissa's plan was significantly increased to $1,414,710, providing funding for appropriate housing supports, assistive technology, daily living assistance, allied health services, and the specialised supports required to transition safely from hospital into the community.

When Luke first engaged with UConnect Coordination, he had only $4,850 remaining in his NDIS plan. With essential supports at risk of ceasing, Luke was facing increasing uncertainty about his future and his ability to continue accessing the services he relied on.
Our Support Coordination team immediately completed a comprehensive review of Luke's circumstances, funding, and support needs. We collaborated closely with Luke, his family, allied health professionals, support providers, and medical practitioners to gather detailed evidence demonstrating that his existing plan no longer reflected his current disability-related needs.
Following this work, we prepared and submitted a Change of Circumstances request to the NDIS. When the initial decision did not adequately address Luke's needs, our team continued to advocate on his behalf by lodging an appeal and providing additional evidence to support his case.
Our persistence and commitment resulted in a significantly improved outcome.
Luke's new NDIS plan increased to $186,420, ensuring he could continue accessing essential daily supports, allied health services, community participation, and Support Coordination without interruption.
This outcome provided Luke and his family with peace of mind and the confidence that his supports were sustainable, allowing him to focus on achieving his goals rather than worrying about funding.
